老年人易疲倦，恐罹骨髓增生不良症候群。一位七十一歲的林先生平日常覺得容易疲倦、沒精神，原本以為是睡眠不足或體力不好的關係而無多留意，直到醫院就醫，經過抽血檢查後發現林先生有貧血的狀況，並做進一步骨髓檢查確認林先生罹患骨髓增生不良症候群，需接受持續性的輸血治療來改善貧血問題。
苗栗市大千綜合醫院血液腫瘤科主任黃明立醫師表示，骨髓增生不良症候群是一種骨髓造血病變，好發年齡多在六十五歲至七十歲前後，造成原因為其三種造血細胞(白血球、紅血球及血小板)中，至少有兩種細胞發生增生不良的型態變化，通常需接受骨髓檢查，透過觀察血球型態，並配合染色體之分析，才能確立診斷。骨髓增生不良症候群的病患在臨床上常會面臨兩個問題：血球偏低造成的疾病，可能會轉化為急性骨髓性白血病，所以可能出現原因不明之貧血；白血球上升或血小板下降，進而導致容易出血、疲倦、臉色蒼白、感染等症狀。
骨髓增生不良症候群治療方式可分為三階段──支持治療方式：大多數病人有貧血的問題，需仰賴輸血，輸血期間常需同時合併接受排鐵治療，以預防體內鐵質過高。
低強度治療方式：包括低強度的化療或免疫調節劑。黃明立醫師說明，近年來有新藥開發，可改善血液問題並延長存活期、提升生活品質及降低轉化成白血病的機會，同時副作用較低，是老年病人可選擇的方式，惟必須事前專案申請才能使用。
高強度治療方式：包括誘導化療及造血幹細胞移植。此方式雖有較高的機會改變疾病的自然進展，但相對的其合併症及死亡率也較高，因此現階段仍屬臨床試驗性質。
黃醫師提醒銀髮族，平時應定期接受健康檢查，如有異常出血、疲倦、體重減輕或查無原因之感染，應盡早至醫院接受進一步的檢查，以期能早期發現早期治療，讓老年生活更加健康有活力。
